     - Full Name
     - Address
     - Quantity of Buttons
     - Your Preferred Method of Payment
     - Button Artwork

I will send you an invoice for your order after all of the information has been recieved. All name and address information is kept strictly confidential.

I would prefer not to be contacted by phone, but if you would like to call, email me, and I can send you my phone number, and let you know the best time to call.
